Develop the clinical judgment required to manage non-responders, appliance complications, phase-two decisions, and long-term patient care.
Course description
Straightforward cases teach the protocol. More difficult cases require clinical judgment.
Level 4 is built around the participants’ own documented cases.
Four weeks before the course, each dentist submits two cases with examination records, photographs, relevant imaging, appliance information, adjustment history, clinical response, and a specific question.
Dr. Bis uses those submissions to shape the course discussion.
Participants work through non-responders, incomplete improvement, appliance-related complications, contributing factors outside the appliance, and decisions about repair, remake, reassessment, co-management, or referral.
The course also addresses phase-two planning, including orthodontic, restorative, equilibration, or maintenance options.
Clinical exercises remain peer-to-peer. Patients do not attend the standard course format.
Learning objectives
By the end of Level 4, participants will be able to:
Apply a structured decision process to the non-responding patient
Distinguish appliance-related concerns from diagnostic, compliance, or comorbidity-related factors
Recognize contributing cervical, sleep, systemic, psychosocial, or degenerative factors
Interpret findings involving disc displacement and joint degeneration
Decide when to repair, remake, adjust, reassess, or refer
Troubleshoot an appliance that has been worn or altered
Present and discuss a documented case with clinical peers
Evaluate phase-two treatment options
Communicate prognosis and uncertainty appropriately
Develop a multidisciplinary referral network
Establish long-term recall and reassessment protocols
Recognize when co-management or referral is indicated

Dr. Agatha Bis has over 28 years of clinical experience with a special focus on TMJ & TMD management.
Known for her expertise in both, diagnosis and treatment, Dr. Bis is a leader in TMD education and dedicated to improving patient outcomes through advanced knowledge and skills.
Case-based experience
Each participant presents documented cases and works through the clinical reasoning with Dr. Bis and the group.
